You must complete this form to secure your booking

In order for us to process your application and book you on the chosen course, we will need a few bits of information and a copy of your ID uploaded.

Guidance on Accepted ID

You must provide a valid (in date) form of ID. All images should be clear and readable.

Preferred form of Identification

A Valid Driver Qualification Card (DQC) – This is your blue CPC card and is our preferred form of ID as it also confirms that you are eligible to attend the training. Please upload images of both front and back.

Alternative forms of ID

A Valid Photocard Driving Licence – Please make sure this is valid and you upload images of both the front and the back.

A Valid UK Passport – Please make sure this is valid and also provide the front of another form of photo ID such as your DQC or Driving licence (even if these are no longer valid)
